Michigan Alcohol Screening Test (MAST)

This test is nationally recognized by alcoholism and drug dependence professionals.  You may substitute the words "drug use" for "drinking" to get an initial self-assessment of whether or not you should seek a professional assessment of your drinking or drug use or further help.  Keep track of your answers on a separate piece of paper numbered for each question or print this out and circle your answer to each question.

13.  Have you ever neglected your obligations, your family or your work for two or more days in a row because you were drinking?     Yes     No
